Academic Dismissal Appeal Form

Students who are required to complete an Academic Dismissal Appeal Form have been instructed to submit this form to the Warren Advising Office. Do not submitted this form unless you have been instructed to do so.

1. Fill out the Academic Dismissal Appeal Form (pdf).
2. Mail the completed and signed form along with any additional documentation to:

Authorization for Release of Information

Student information is restricted; the release of any student information is governed by rigid state and federal laws. Any information on file must remain confidential and may not be released to or discussed with anyone unless the student has specifically requested in writing the release of any, or part of, this information. If you would like to authorize a family member, guardian, etc. to have access to your information, you are required to fill out the Authorization for Release of Information (pdf) form and turn it in to the Warren College Advising Office, located on first floor of the Computer Science and Engineering Building (EBU3), Room 1137.

Warren College Individualized Studies Major

Warren College offers an Individualized Studies Major. This offering allows a student to pursue a coherent course of study that is not formally recognized as a major at UC San Diego but would otherwise meet all of the standards established by the College and by the Academic Senate. Interested students must submit a written proposal explaining the merits of an individualized major and why their educational goals cannot be accommodated within an existing major. The proposal must be approved by a faculty advisor and then approved by the Warren College Provost/Executive Committee. Application Guidelines (pdf)

Readmission Evaluation Form (pdf)

Students who were academically disqualified and are now seeking readmission must submit this form, along with supporting documentation, to the Warren Academic Advising Office.

Self-Assessment for Students Experiencing Academic Difficulty (pdf)

This form is intended to help students identify issues that may be resulting in academic difficulty.  You may also bring the completed form to an advising session for further assistance.
